Important:

• All applicants must complete the following forms in order to participate in the Elaine Sortino Softball School:
   
  • Participant Release of Liability
  • Medical and Immunization History *
     
• All applicants must mail a key deposit check payable to "Umass Conference Services" for $50.  This check will be returned at the end of camp upon return of room key.
   
• Forms and key deposit check must be completed and mailed to the following address IMMEDIATELY to finalize registration:

• Players without the proper forms will not be permitted to participate in camp. Registration will not be full confirmed until all health and liability forms are completed and received by the Elaine Sortino Softball School.

* Participants must have a documented doctor's physical completed within one year of the start date of the softball school.  There are no exceptions to this requirement.
