scheduling instructional time
for individuals, groups of 2 or
more, or teams. It is also a
tool to request reservations for
batting cage or full facility
rentals.
Planet Fastpitch utilizes a
monthly scheduling calendar.
Please complete the form with
the most flexibility and ranges
of days and times as possible.
We will attempt to make your
first choice available for you.
Please note that this is a first
come, first serve scheduling
system. We do our best to try to
fit everyone into the schedule
at times that are convenient.
You must have a current
liability waiver on file at PFP
to participate in any activity.
Please print, complete and carry
or mail this form to PFP. Each
member of a team must have a
waiver signed before
participation in any activity is
permitted.
